정서반응성과 감정표현불능증이 신체화에 이르는 경로: 긍정, 부정 정서 차원의 탐색적 접근

Pathways from emotional reactivity and alexithymia to somatization: An approach based on positive and negative affective valence

초록

본 연구는 긍정 및 부정 정서 차원에 따라, 정서반응성과 감정표현불능증이 우울을 매개로 신체화를 설명하는 최적의 경로를 탐색하고자 하였다. 이를 위해 만 19세 이상 성인 1,001명을 대상으로 온라인 조사를 실시하였고, 이 중 우울(CES-D) 20점 이상 또는 신체화(PHQ-15) 5점 이상인 712명을 준임상군으로 선정하였다. 확인적 요인분석을 통해 측정모형의 타당성을 검증하였고, 이후 연구모형과 경쟁모형을 대상으로 모형경쟁을 실시하였다. 연구모형은 정서반응성과 감정표현불능증의 상호작용이 우울을 매개로 신체화에 영향을 미친다는 조절된 매개모형으로, 경쟁모형은 정서반응성과 감정표현불능증이 두 개의 독립변인으로서 우울을 매개로 신체화에 독자적인 영향을 미친다는 다중 독립변수 매개모형으로 상정하였다. 연구 결과, 측정모형의 타당성이 검증되었고, 긍정, 부정 정서 차원 모두 경쟁모형이 연구모형에 비해 모형적합도가 우수하여 최종모형으로 채택되었다. 구체적으로, 부정 정서반응성과 부정 감정표현불능증, 긍정 감정표현불능증은 우울을 매개로 신체화를 심화시켰고, 긍정 정서반응성은 우울을 통해 신체화를 완화시켰다. 상기 결과를 토대로 본 연구의 의의와 한계를 논의하고, 후속 연구에 대한 제언을 제시하였다.

Abstract

This study examined pathways linking emotional reactivity and alexithymia to somatization via depression, considering positive and negative affective valence. Of the 1,001 adults surveyed online, 712 individuals meeting subclinical criteria on the CES-D or PHQ-15 were included in the analyses. Following measurement model validation via confirmatory factor analysis (CFA), the hypothesized moderated mediation model, which tested the interaction between emotional reactivity and alexithymia, was compared against a competing model specifying their independent and distinct contributions. Across positive and negative affect, the competing model demonstrated superior fit and was retained. Specifically, negative emotional reactivity and both negative and positive alexithymia exacerbated somatization through depression, whereas positive emotional reactivity mitigated it. Theoretical and clinical implications, along with limitations and future research directions, are discussed.
